This calendar I have for each student to note whether they have forgotten a pencil or other materials, misbehaved, or any other thing I can document quickly that is not a school offense. 

This way I can know the exact day and offense of the student and if it requires further documentation or inspection, I have it ready!

 

This is a behavior analysis form for more serious behaviors for documentation.

 

This log is for students who may continually misbehave so I can have a list of their actions or behaviors for quick reference. 

 

Then I have this page that every student fills out the top of so I know contact information and emergency info. 

Other pages to be added as the year goes on is information gathered from ice breakers, important assessment data, and other student info.
